Use this form to request a CES badge replacement. There is a $20 replacement fee. Replacement badges are printed at CES Central Office, 40 Lindeman Drive, 2nd Floor.
In an effort to better coordinate our response to the technology and maintenance issues that may arise in the course of your daily work, we have a ticket system. Submit a ticket for all technology and maintenance needs. Technology requests will be received by our IT staff and Maintenance requests will be received by our Facilities staff. They will review and determine who the appropriate person in the organization is to respond to the request.
